Disability Claim
For veterans looking to file a disability claim, please bring the following items with you to your appointment:
- Private Medical Records regarding conditions you wish to file for
- If you are filing for PTSD or another mental health condition for the first time, the VA Form 21-0781 is required. Click here for the form
- Birth certificates and SSN cards for dependents (if applicable and they're not already on your current disability award)
- Marriage certificate (if applicable and your spouse is not already on your current disability award)
- DD214
- NGB 22 (National Guard only
BDD Claims
Benefits Delivery at Discharge (BDD) Claims can be filed 90 -180 days prior to an Active Duty service member's date of separation. The list below details the required documents you will need to submit with your BDD Claim.
- Active duty medical records - also referred to as Service Treatment Records (STR)
These can be requested at your Medical Group Outpatient Records office. A disc will be provided and we have the ability to pull the records from the disc. For dental records, visit your Medical Group Dental Office.
- Separation Health Assessment DBQ -
- If you are filing for PTSD or another mental health condition, the VA Form 21-0781 is required -
- Birth certificates and SSN Cards for dependents (if applicable)
- Marriage Certificate (if applicable)
Veterans Pension
Required documents to file for a Veterans Pension:
- DD214
- Statements showing all forms of income - Examples (Social Sec, Pensions, Trust Payment)
- List of medical expenses that you do not receive reimbursement for
Surviving Spouse
Dependent Indemnity Compensation:
If you are the surviving spouse of a veteran, you may qualify for this benefit. In most cases, the veteran must have passed away from a service connected condition, or held the disability rating of 100% Permanent and Total for at least 10 years prior to their death. There are other situations a surviving spouse may qualify. Make an appointment with our office to discuss details.

Veterans Burial Information
If you wish to file for burial reimbursement for a veteran, the following documents are required and should be brought with you to your appointment with a Veterans Service Officer:
